I was just listening to "The World, Tech Podcast" #213 (from Oct 7th I think)
In episode 213, the host, Clark Boyd, talks to the folks from OpenMRS.org (Open Medical Records) - made me think of your post here. I'm guessing they're already on your radar, but if not, give a listen. It sounds like they'd be a good partner for your ID system.
